Formula basis

Age points (35-44=30, 45-54=38, 55-64=45, 65-69=51, 70+=53) plus -7 for female, 5 for current smoking, 6 for diabetes, blood pressure category (optimal=-7, normal/high-normal=0, stage 1 hypertension=4, stage 2+ hypertension=6), LDL category (five levels from under 100=0 to 180+=11), HDL category (under 40=0, 40-59=-5, 60+=-6), and CKD category (stage 1-2=0, stage 3=3, stage 4-5=14) are summed. The total score is then mapped to the published 10-year incidence probability table (35 or below = under 1%, above 71 = over 28%) from the original paper.

Estimates a 10-year coronary heart disease (e.g. myocardial infarction) incidence rate from a points-based score using age, sex, current smoking, diabetes, blood pressure category, LDL cholesterol category, HDL cholesterol category, and chronic kidney disease (CKD) stage.

Exclusions and checks required

  • This reproduces a simplified point-sheet version of the underlying Cox proportional-hazards statistical model; it does not guarantee an individual's exact incidence probability.
  • The underlying cohort was drawn from an urban population in Suita City, Osaka, which limits generalizability to other populations.
  • Current smoking is scored as 5 points, the value stated in the published point table. Implementations that use a different weight will not produce the same total score.
  • Blood pressure, LDL, and HDL are entered as categories, not raw measured values, so you must select the band that applies to you.
